Guiding you through life

One visit at a time

The journey near the end of our lives can be a daunting experience to navigate through. At times we may go through these experiences alone, without family, without support. Yet there are many different end-of-life care options that is made available to your loved ones. They deserve quality care and compassion at the comfort of their own home.

Grace Hospice of Nevada (an affiliate of CHS Hospice) strives to preserve the dignity of life to all that we serve. We aim to provide comfort and support to all patients in their time of need.

Services We Provide

 FAQs

  • Hospice aims to provide care, comfort and quality of life to people nearing the end of life. A team of skilled clinicians provide this care at the comfort of one's own home or in a nursing facility.

  • A loved one qualifies for hospice when their physician agrees that their life expectancy is six months or less. They also qualify if their physician confirms that they must be homebound and require skilled nursing and/or therapy services.

  • Routine Care - Regular home visits to assist pain and symptom management. These visits can include therapy services (physical, occupational, and/or speech) as well as social workers. This level of care can provide medication, equipment and education to patients.

Continuous Home Care - Round-the-clock and/or extended periods of care for the patient. Further skilled nursing is provided at home to help ease pain and offer support for longer periods of time.

Inpatient Care - Skilled nursing provided in an inpatient facility, nursing home or hospital. This crisis-like level of care is short-term and aims to manage out of control pain and/or symptoms.

Respite Care - Temporary care provided in an inpatient facility, nursing home or hospital. This alleviates a family or friend's role as a caretaker and helps give them time off.
