SKILLED NURSING SERVICES

BASIC NURSING CARE - We provide a wide array of nursing care that includes assessment of your general well being, monitoring of vital signs, and simple tests that can be performed at your home. Our nurses regularly reevaluate your needs to create a treatment plan tailored to the type of your medical condition.

WOUND CARE - We have clinicians who are highly trained to perform wound management of acute or chronic wounds.  Our nurses can teach care givers how to provide wound care independently and safely.

MEDICINE ADMINISTRATION - We help you organize and pre-pour your medications and collaborate with your doctors in performing medication reconciliation. We provide medication education including side-effects to watch for. We also offer IV therapy at home.

REHABILITATION

PHYSICAL THERAPY - Our physical therapists will design a plan of care and select appropriate treatments to optimize functional ability in a safe home environment.

OCCUPATIONAL THERAPY - Our occupational therapists will design programs that will help you regain and improve the skills to perform activities of daily living, minimizing residual physical disability after an injury.

SPEECH THERAPY - Our speech therapists are instrumental when communication becomes a challenge in your daily living. They are experts in regaining your ability to speak or use alternative means of communication. They also help develop exercises to strengthen muscles used for swallowing and prevent aspiration.

SUPPORT SERVICES

MEDICAL SOCIAL - Our licensed social workers will assess your support system and help identify available resources that play a significant role in your recovery. They will also guide you on how to access community resources and other government benefit programs.

HOME HEALTH AIDE - Our home health aides are under a supervision of a registered nurse. They will assist you in bathing, grooming, and other basic nursing care. They are instrumental during the transition process while you are recovering or your caregiver is learning how to be independent in taking care of you.
